When chronically ill person is discharged from the hospital and returns home, they are often overwhelmed with the challenges of new physical conditions, new medications, and new care instructions. Whether they are dealing with a fall, a stroke, diabetes, COPD, hypertension, asthma, arthritis, or a combination of illnesses, the sooner they can learn how to self-manage their circumstances, the more quickly they will have improvements in their health and their lives.
The Extended Health Community Programs (EHCP) offered by the Piedmont Triad Regional Council Area Agency on Aging (PTRC AAA) give proven results to help people get back in charge of their lives.
A Matter of Balance™ improves health and balance for those at risk of falling. Participants are given a Personal Home Assessment Checklist and learn practical strategies for reducing the fear of falling and reducing fall risk factors.
Living Healthy™ is a Living Healthy workshop, a six week Chronic Disease Self-Management Program (CDSMP) course where participants learn proven ways to maintain stamina, improve strength, and manage use of medications.
Living Healthy With Diabetes™ is a six week workshop that helps seniors stay on top of the care program, learn better self-care methods, and avoid going back to the hospital.
Tai Chi For Arthritis™ reduces pain and stiffness while improving function. The ancient discipline of Tai Chi combines agile steps, joint-safe exercise, and mental strength to improve mobility, breathing, and relaxation. A two-day training to become a Tai Chi instructor will be offered October 13 and 14 at the PTRC Area Agency on Aging in Kernersville.
